National Newfoundland Rescue Inc. (“NNR”), a not forprofit corporation organized and existing under the laws of the State of Connecticut, with theprinciple and registered office address at 1579 Route 12, Gales Ferry, CT 06335, herein referredto as National Newfoundland Rescue Inc., NNR or Rescue.Foster, being of lawful age, hereby expressly releases National Newfoundland Rescue Inc., its licensees, agents,successors and assigns from all liability for claims and/or demands arising out of the agreementas set forth below.
FOSTER HAS READ THE FOREGOING FOSTER CONTRACT AND FULLY UNDERSTANDS AND AGREES TO BE BOUND TO ITS PROVISIONS. BY TYPING YOUR NAME AND TODAY’S DATE IN THE SIGNATURE BOX BELOW, YOU AGREE TO BE BOUND TO THIS AGREEMENT.
Foster agrees to attend NNR foster orientation that provides specific guidelines on how to safely and successfully foster a dog. Topics in this orientation include instructions on loss prevention, safe handling, what to do in emergencies, how to properly introduce a new dog to your home, family and existing pack, and much more.
Media Release: Foster hereby authorizes National Newfoundland Rescue Inc. to use pictures, record , voice, photographs, film, audio and video, to edit these recordings at its discretion, and to incorporate these recordings across its social media platforms, and to use such recordings, movie and sound films, audio and videotapes, and broadcast programs in any manner of media whatsoever, including unrestricted use for purposes of publicity, educational materials, advertising and promotional materials and to use Foster’s likeness, voice and biographic or other information in connection therewith for perpetuity across all its social media platforms and its website. Foster further agrees to indemnify and hold harmless National Newfoundland Rescue Inc., its licensees, agents, successors and assigns, from any and all claims and liability for damages, losses or expenses of any sort arising from the making of such recordings and their use. Foster further acknowledges that, there were no promises of any compensation for such use by National Newfoundland Rescue Inc. or by anyone associated with National Newfoundland Rescue Inc. and, that National Newfoundland Rescue Inc. exclusively owns all rights to these recordings regardless of the form in which they are produced or used.
Foster agrees not to transfer custody of the dog to anyone other than NNR and agrees to return the dog to Rescue upon demand.
Foster understands that Rescue will find this dog a home and that Foster is not to adopt or give away this dog to anyone not approved by rescue.
Foster agrees to exercise normal care in maintaining the health of this Dog.
Foster agrees to contact NNR immediately if the dog becomes ill or is missing.
Foster agrees all medical care/treatment needs to be authorized by Rescue before the expense is incurred unless the situation can be categorized as life-threatening. Life threatening means that the dog is in danger of dying. Expenses incurred without approval will not be paid for by Rescue and the Foster will be responsible for covering these expenses.
Foster will provide fresh food, fresh water, and appropriate exercise daily.
Foster agrees to provide updates to Rescue on the dog’s progress.
Foster agrees to always keep the foster dog within a fenced yard (not left outside on a chain) or supervised on a leash. Foster agrees to never leave this dog outside when no one is home; to never allow the dog to be tied to a doghouse or left out on a chain, or left unsupervised in a yard.
Foster agrees to not allow a minor (person under the age of 18) to walk or handle the dog unsupervised by a parent.
Foster is NEVER to use the following tools on the dog: Flexi/retractable leash Any type of electronic collar Prong collar or choke chain Any tool deemed aversive by the foster support team
Foster has been advised that dogs can sometimes dig, chew, soil, and exhibit other undesirable traits that could cause damage to personal property. Rescue is not responsible for damage caused by the dog.
The rescue has disclosed all known behavioral and medical issues of the dog, but does not guarantee the accuracy of this information or lack of information from the dogs original owner.
Foster agrees to strictly follow any post-operative wound care or any other care guidelines outlined by the veterinarian including but not limited to: # of days the dog should wear a surgical cone, restrict exercise, stay dry, stay clean, and follow any other advice from the veterinarian. I understand that if I deviate from this advice I am financially liable for the consequences.
Foster agrees that if foster intends to adopt their foster dog, they have first consideration but need approval from Rescue.
Foster agrees that adoption will be completed within 48 hours of the dog being deemed by the rescue as ready for adoption. Completing adoption requires signing the adoption agreement and submitting the adoption donation.
It is further agreed that if the Foster fails to perform any of those acts which he/she has guaranteed to perform, that the Rescue may, at its option, revoke this agreement and retake possession of the dog without delay. Foster agrees to hold harmless and release Rescue and/or its members, volunteers and other individuals working for and/or assisting with rescue from all liability, claims, and/or causes of action arising from any acts, injury and/or occurrences engaged in and/or caused by the dog subject to this agreement, as well as indemnification for damages, court costs, and/or attorney's fees incurred by the Rescue and/or its members for any court proceedings relating to such matters.
By completing this form and submitting it to National Newfoundland Rescue Inc. you (Foster) are providing a legally binding electronic signature, indicating that you agree to adhere to all of the provisions in this Foster Contract.
