Volunteer Application

I hereby agree to foster the animal from Every Last One Rescue under the following terms and conditions:

Assumption of Risk: I understand that fostering an animal involves inherent risks, including but not limited to, the risk of injury to myself, members of my household, and the animal. I voluntarily assume all such risks and agree to hold Every Last One Rescue, its directors, officers, volunteers, and agents harmless from any and all liabilities arising from my participation in the foster program.

Release of Liability: In consideration of being permitted to foster an animal from Every Last One  Rescue I hereby release, waive, discharge, and covenant not to sue Every Last One, its directors, officers, volunteers, and agents from any and all claims, demands, causes of action, damages, or liabilities, including but not limited to those arising from negligence, for personal injury, property damage, or death that may occur as a result of my participation in the foster program. The Individual agrees never to bring a claim or suit against Every Last One Rescue related to the Individuals interaction with ELO Dogs or ELO volunteers. This Waiver shall also be binding upon the Individual’s spouse, heirs, executors, personal representatives, successors and assigns.'

The individual agrees that ELO may use any photograph of the Individual and any ELO Dog taken by ELO or its agents or volunteers, or provided by the Individual to ELO, in promotional materials. The Individual consents to ELOs use of the Individuals likeness in such promotional materials, and specifically releases ELO from any claim arising out of ELOs use of the Individuals likeness or photographs of the Individual in such promotional materials.

Indemnification: I agree to indemnify and hold harmless Every Last One,, its directors, officers, volunteers, and agents from any and all claims, demands, causes of action, damages, or liabilities, including but not limited to those arising from negligence, brought by third parties arising out of or in connection with my participation in the foster program.

Emergency Medical Treatment: In the event of an emergency, I authorize Every Last One to seek emergency medical treatment for the foster animal named [Animal's Name]. I understand that I will be responsible for any costs associated with such treatment.

Care and Responsibility: I agree to provide proper care and supervision for the foster animal, including but not limited to, feeding, grooming, exercising, and providing necessary medical treatment as directed by Every Last One. I understand that I am responsible for any damages caused by the foster animal while in my care.

Return of Animal: I agree to return the foster animal to Every Last One upon request or at the end of the foster period. I understand that Every Last One reserves the right to reclaim the animal at any time, with or without cause, upon written notice to me.

Governing Law: This agreement shall be governed by and construed in accordance with the laws of New York.

I have read this Foster Agreement Liability Waiver and fully understand its terms and conditions. I acknowledge that I am signing this agreement voluntarily and without any inducement or assurance of any nature.
